Stepping Down From Care At Haven

Being discharged from Haven Behavioral Hospital is a big step. You may still feel overwhelmed or unsure about what comes next. The last thing you need is to be hanging on a waitlist for long-term care. Our goal is to support you during this transition, providing direction, compassion, and stabilization skills you need to be safe.

We’re specially trained to support individuals stepping down from intensive treatment programs. Here’s how we can help:

  • Telehealth Convenience: All sessions are online, allowing you to connect with us from the comfort of your home.
  • Non-Judgmental Listening: Share your experiences, feelings, and concerns without fear of judgment or overreaction.
  • Focused Support: We’ll work with you to create a personalized plan to manage stress, build coping skills, and maintain stability.
  • Preventing Rehospitalization: Our goal is to help you develop the tools to prevent future crises and avoid returning to the hospital.

In your first session, we’ll begin with a therapeutic assessment to understand your needs, challenges, and goals. If you’re facing immediate concerns, we’ll develop a crisis plan and provide stabilization services to ensure you are secure and supported. Our no-waitlist, short-term therapy model ensures you receive the stabilization and care you need right away. If ongoing therapy is required, we’ll continue working with you until we can transition you to a long-term therapist, making sure you have the consistent support you deserve every step of the way.
